Benefits at SAM, Inc. Our employees are our most valuable assets. We look to create long-term relationships with our employees, and in support of this, our benefit program is designed to assist them and their dependents in maintaining healthy and prosperous lifestyles. Below are a few of the many benefits we provide to all full-time employees.

	Medical, Dental, Vision and Prescription Coverage All full-time regular employees are eligible to participate in our medical, dental, vision and prescription coverage. Our Blue Cross/Blue Shield Program includes both HMO and PPO plans. The company pays for a portion of the premiums. Premium deductions are made on a pre-tax basis.
	Basic Life and AD&D Insurance We provide employees with life insurance and accidental death and disability coverage in the amount of one times annual salary to a maximum of $100,000 at no cost. Employees may elect to purchase additional life insurance coverage for themselves and their dependents.
	Disability Insurance Long-term disability insurance coverage is provided by SAM, Inc. Short-term disability insurance may be purchased by employees in varying amounts at their own expense.
	Paid Time Off (PTO) We fully recognize that employees have a life outside of work. To help balance professional and personal needs, we offer paid time off to cover vacations, sick time and personal time. Amounts of PTO increase with tenure, and portions may be carried from year to year.
	Holidays SAM, Inc. observes six designated holidays and provides four additional floating holidays.
	401(k) Plan To help employees achieve long-term financial goals, we provide a 401(k) retirement plan, which includes company matching and discretionary contribution to further supplement retirement dollars.
